Registering with a GP practice

The Practice is happy to accept patients wishing to register with the practice provided they are eligible and are living within the practice area.

To apply to register with the practice please contact one of our reception staff.  You will be given an application form and a new patient questionnaire which must be completed and returned.  You can pick up the forms from reception or print them off and hand in to the surgery.  Please complete both forms.

  • Patient Registration Form
  • New Patient Questionnaire

You will need to bring the forms into the practice with proof of identity and address so please don’t post them in as we can’t process them without having had sight of the relevant ID.

Our reception staff will arrange an initial consultation with the Practice Nurse. 

Note that if you move out of the practice area, you will need to register with a new practice as soon as you move.  We can provide information about how to find a new practice.

Registering as a Temporary Resident

You can only register with a GP practice if you’re staying in the area for more than 3 months. If you’re not, you may still be treated but as a Temporary Resident.

You can also use the Access to Healthcare. This is a GP registration card to help you register with a GP Practice. The card gives information on your rights to accessing healthcare. You can find the card on the Health Literacy Place website.
