How to access your GP record

Your medical records hold information about you, including information like any conditions or allergies you may have and any medicine you're taking.

You can make an application to see your own records, this is called a subject access request (SARS).  You can also authorise someone else to make the application and to look at them for you.

Please complete the Access to Your Health Records form and hand it in to the surgery.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else.  Please contact our staff who will be happy to assist you with this.
